Title: Occupational Therapist

What You’ll Do:

  • Develops occupational therapy plan of care with realistic/measurable outcomes in collaboration with the client/family and physician including discharge goals and according to Organization protocols.

  • Provides education and promotes client’s rights and responsibilities for informed decision making in the plan of care.

  • Develops, implements, and revises a plan of care that incorporates goals consistent with the client’s age, condition and needs.

  • Implements client care plan activities based on treatment goals towards independent living for the client/family.

  • Continually assesses the client’s needs for occupational interventions and evaluates the effectiveness of the interventions on the occupational visit report.

  • Refers to internal and external community resources to meet client outcomes when appropriate.

  • Initiates diagnostic, preventive and rehabilitative procedures as appropriate to the client’s care & safety to assist in the development and revision of Plan of Care.

  • Evaluates and documents progress toward outcomes and coordinates discharge plan with client and family and other health professionals.

    What You’ll Bring:

  • Master’s and/or Doctorate in Occupational Therapy from an accredited institution.

  • One-year clinical Occupational Therapy experience preferred. Home Health experience also preferred.

  • Current O.T. License in State of Maine & current CPR Card.

  • Must have a reliable and insured automobile and a current Maine driver’s license.
