Company Overview and Mission Columbus House opened its doors in 1982 to provide services for men and women at least 18 years of age. Our goals quickly broadened from the mere provision?of food and shelter and simple survival, to understanding and working toward overcoming the problems which cause people to become homeless.

Objective

To support and link consumers to housing, educational, employment, vocational, spiritual, recreational, medical, and treatment resources; to reduce the numbers of men and women in Middlesex County who struggle with mental illness and/or substance abuse which has or can lead them to homelessness.

Qualifications

  • BA degree required, Masters preferred or four years plus experience working as a case manager in outreach/substance abuse position.
  • Experienced individual to work with people who are homeless and who have a history of mental health issues and substance abuse and who are currently disengaged from services.
  • Knowledgeable of outreach activities, engagement skills, basic entitlements, housing, state and community resources and mental health issues.
  • Must have organizational and network skills to create linkages to medical, mental health, substance abuse treatment, employment, and educational opportunities;
  • Sensitivity to working with people with various cultural backgrounds and must have knowledge of Trauma Informed and Gender Specific care services.
  • Computer literate with the ability to learn new software programs required.
  • Must have valid Drivers License and a good driving record. Harm Reduction skills and PTSD knowledge a plus. Bi-lingual English/Spanish preferred.

Responsibilities

  • Engage in street outreach to consumers who have histories of homelessness, mental illness, substance abuse, unemployment, incomplete educations, lack of employment skills and are disengaged from services.
  • Must be comfortable providing a full range of case management services to individuals in the community, i.e. in soup kitchens, shelters and on the streets.
  • Must team with and support all other outreach case managers and shelter case managers as appropriate.
  • Attend all required network and staff meetings; meet with Middlesex and Transitional Programs Manager for regular supervision; review caseload on a weekly basis; complete required monthly, weekly and daily charting and movement reports.
  • Position will be required to collaborate with the CAN Entry and Exit process in Middlesex region.
  • Must be able to work one evening a week at the local Warming Center during winter season after 9PM.
  • Complete all required documentation accurately and in timely fashion.
  • Enter all required client data completely, accurately and timely into Case Worthy computer program.
  • Other related duties as assigned.

Supervised by: Manager of Middlesex Services

Salary Range:$19.23-$21.23 per hour plus excellent benefits

Schedule: Monday - Friday 40 hours; 8am-4pm some evenings
