Catholic Health Initiatives Clinical Pharmacist in Mount Sterling, Kentucky

Overview

Saint Joseph Mount Sterling is a not-for-profit faith-based hospital located in Mount Sterling Kentucky. The hospital was founded in 1918—formerly known as Mary Chiles Hospital—and now serves as a 42-bed community hospital for the residents of Bath Bourbon Clark Menifee Montgomery Nicholas Powell and Rowan counties. JOB SUMMARY / PURPOSE

Provide accurate, safe, and prompt medications and IV service as needed for adequate patient care according to professional standards and practices. Serves as a resource person and a clinical liasion to physicians, pharmacists, nurses and other hospital personnel. Provides pharmaceutical care services according to professional standards and practices. Provide care to patients of all ages. Provide patient care/services based on identified needs/developmental level for all patient populations. Care/services may include assessment of individual patient need, incorporation of age specific characteristics in patient education, communication, and planning; provision of individualized care relevant to patient needs; and recognition, monitoring, and provision of interventions as appropriate. Facilitates public relations between pharmacy, nursing, and medical staff.

ESSENTIAL KEY JOB RESPONSIBILITIES

  1. Documents activities consistent with Hospital/Department/Unit Guidelines.

  2. Exhibits mastery of clinical initiatives, pharmacokinetics, anti-coagulation, and nutrition. Exhibits competency in clinical microbiology and therapeutic areas consistent with practice.

  3. Actively monitors drug therapy and provides pharmacist clinical interventions according to assignment and as identified by patient needs.

  4. Provides therapeutic drug monitoring consults, provides drug information consults and provides patient consultation/education as identified and as requested.

  5. Participates in clinical rounds and interdisciplinary patient care activities as directed by Pharmacy Manager/Director.

  6. Actively prevents inappropriate medication use and inappropriate medication therapy. Intervenes to assure appropriate care to assigned patients.

  7. Monitors high risk patient populations for appropriate medication use and assures Core Measures are met on appropriate patients.

  8. Demonstrates competency in maintaining computerized medication profiles and processing medication lists for cart filling and IV admixtures.

  9. Maintains the operation of the unit dose area, IV room, and satellite pharmacy effectively. Competent dispensing medications according to procedures and laws.

  10. Supervises technical personnel when staffing. Provides appropriate feedback to technical personnel and discusses with Pharmacy Manager/Director when necessary.

  11. Evaluates appropriateness of medication orders for patients of all ages, diseases, and conditions. Responds to inappropriate orders appropriately.

  12. Reviews new medications for formulary consideration. Meet with representatives of pharmaceutical companies as requested.

  13. Participates in medication error avoidance programs and routinely reports medication errors and adverse events

  14. Other duties as assigned.

Qualifications

Required Education for Staff Job Levels: B.S. or Pharm D. degree in pharmacy

Required Licensure and Certifications: Licensed by the Kentucky Board of Pharmacy

Required Minimum Knowledge, Skills and Abilities: Good communication skills, both oral and written. Proficiency with Microsoft Office software.

Pay Range

$49.08 - $71.16 /hour
