The Link Case Manager - Dignity House in Minneapolis, Minnesota

Description Mission: The Link works with youth and young families to overcome the impacts of poverty and social injustice. Our 23 programs provide supportive housing, intensive case management services, and skills development to youth at risk of or experiencing homelessness, sex trafficking and/or involvement in the juvenile justice system. The Link prides itself on its innovative, youth-centered, strengths-based programming for youth and families as well as programs that are co-designed, implemented and overseen by youth. Position Specifics Hiring Salary Range: $$23.13-$26.00/hour based on qualifications Job Type: Full-Time, Non-Exempt Location: Crystal, MN Hours/Days: Mon-Fri, 9:00-5:00 Note: This position requires driving. To be able to drive on behalf of The Link, candidates must have a valid driver's license, be able to successfully clear a criminal and motor vehicle background check, and maintain personal insurance at the levels of $100,000 Bodily Injury/Per Person and $300,000 Bodily Injury/Per Accident. Job offer is contingent on having these requirements. According to Minnesota State Administrative Rule 2960.0100, Subp. 6,C, employees of Children's Residential Facilities must be at least 21 years of age. BASIC FUNCTION This position is responsible for the case management for The Link s Emergency Shelter for minors, Dignity House. The Case Manager will ensure high quality case management and programming, including groups and social and recreational activities. The Case Manager is responsible for working directly with minors, ages 10-17, who have experienced homelessness, trauma and disenfranchisement and who will need high level case management supports to assist them in progressing toward their goals and accessing resources and services needed to stabilize and recover. The position will also require that services and supports for the children of any youth of the residents be provided through on-site or referrals. In addition to case management the Case Manager is responsible for developing, supporting and implementing youth programming that is culturally competent and grounded in positive youth development, harm reduction, and trauma-informed care. Documentation of demographic information will also be required of this position as determined by The Link and our funding sources. Participation in community and/or state level meetings may also be required as needed. ESSENTIAL FUNCTIONS Develop Individualized Service Plans for 5 youth in shelter, track and document incremental progress toward the achievement of goals. Support and advocate with youth in the community Meet daily/weekly with youth to identify service needs and provide support Provide clear and daily communication with the Dignity House Program Manager and other team members to ensure holistic, consistent supports and services are in place Ensure case management supports focus on safety, health, recovery, and stability and individualized goals Provide one-on-one and group supports Complete progress/case notes and maintain participant records. Provide crisis intervention services during regular work hours as needed. Along with Dignity House Program Manager, research, and implement program improvement and expansion and identify trends in population or service interventions Create and maintain a community and culture that is warm, welcoming and supportive of all youth 24-7 Develop collaborations and broker services for youth beyond the scope of Dignity House Provide support services around family reunification, when possible Participate in committees as appointed internally and in the community Collect and document all required demographic and outcome information Complete and assist with funding reports Responsible for being a part of the on-call rotation. This includes, but is not limited to, covering shifts, responding to crisis calls, working non-traditional hours; Ie. Weekends, evenings, etc. (schedule will be adjusted to ensure fair time off.)
