Job Description

Guides staff in the performance of duties. Handles complex patient care coordination cases. Performs all functions of a resource coordinator.

Job Responsibility

  • Provides guidance to team members in the performance of duties; performs all functions of a resource coordinator.

  • Serves as a resource for difficult or complex care coordination issues.

  • Interviews candidates, family members and caregivers to compile and document relevant information (demographic, economic, social, institutional, etc.).

  • Describes available services and advantages of various care management activities; provides program information and materials to candidates.

  • Assists members in scheduling appointments for consultations, diagnostic testing, program meetings, etc.

  • Maintains integrity of related records in accordance with system policies and procedures and state and federal regulations.

  • Develops and generates reports summarizing program activities, as required by management.

  • Completes related assessment; performs administrative assessment of candidates' eligibility for social services and care management programs.

  • Serves as a point-of-contact to program members, family members and caregivers.

  • Encourages use of program resources to maximize wellness and independence.

  • May travel to member's home, medical appointments, and/or meetings to provide non-clinical support, as directed.

  • Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions.

Job Qualification

  • High School Diploma or equivalent required. Associate's Degree preferred.

  • 3-5 years of relevant experience, required.

  • Bachelor's or Master's degree in relevant field strongly preferred.

  • Health home or care management experience strongly preferred.

  • NYS Driver's License strongly preferred.

The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).

The salary range for this position is $47,476-$75,012/year
