Responsibilities of the position include, but not limited to skilled assessments, care plans and care plan implementation, patient education and clinical evaluation or interventions. This position requires working with medical professionals who are working under time and efficiency constraints in a busy ambulatory care setting with a patient centered care focus on quality, safety, and service.
