Maintain integrity of Electronic Medical Records (EMR) in all mediums according to department policy & procedures. Description Opens & sorts incoming mail. Obtains hospital discharge records & uses hospital discharge list to reconcile receipt of paper charts. Prepares the paper medical record for scanning. Scans & indexes paper documents into the EMR. Electronically analyzes, reanalyzes, & assigns signature deficiencies to the appropriate practitioner in the Chart Completion Module. Performs quality reviews of scanned & interfaced images in the EMR & verifies the quality of image, accuracy of patient name, appropriate bar code, account number & document type. Resolves issues with scanned & interfaced documents. Maintains Turn-Around-Time (TAT) for prepping, scanning, analysis, & quality control of documents. Sorts & scans loose sheets into EMR accurately daily. Retrieves, signs out, & prepares medical records as requested for patient care, correspondence, studies, or audits. Performs file maintenance & prepares records for offsite storage. Responds to requests for patient information in a timely manner. Communicate with internal & external customers requesting patient information. Processes & files Advanced Healthcare Directives in accordance with Federal &/or State Statutes & Hospital/Departmental procedures. Consistently provides service excellence to all patients, family members, visitors, volunteers & co-workers. Performs other job-related duties & assignments as requested. Qualifications EMRS Trainee - Grade S7 Job Code 8197 Ability to read, write & communicate in English. High School Diploma or GED. Upon completion of the CCH EMRS Training program, the incumbent must pass the basic Medical Terminology course or pass the CCH Medical Terminology exam. Excellent customer service skills with the ability to communicate efficiently. Upon completion of the CCH EMRS training program, the incumbent must pass the Cape Cod Hospital Medical Record Department Qualifying test. With the successful completion of the CCH EMRS Training program, incumbent will transfer into EMRS Grade S8, Job Code: 8224. EMRS - Grade 8 - Job Code 8224 Ability to read, write, & communicate in English. High School Diploma or GED. 12 months experience utilizing an Electronic Medical Records (EMR) system OR. successful passage of the Medical Record Department Qualifying test. Successful passage of basic Medical Terminology course OR successful passage of the CCH Medical Terminology exam. Basic knowledge of Microsoft Word, Excel, & Outlook. Excellent customer service skills with the ability to communicate efficiently. Exceptional organizational skills with attention to detail. Ability to problem solve, organize, & prioritize workload to meet turn-around time benchmarks. Schedule Details: 40 hours per week, 7a-3p, rot. days, weekends & holidays
