Amergis RN/LPN Psych Case Manager in Danvers, Massachusetts
Amergis Staffing is seeking a Nurse Case Manager (RN or LPN) to work in the community with adults with severe and persistent mental illnesses. The ideal candidate would possess knowledge of psychiatric medications, psychiatric and medical treatment. This person would be comfortable with outreach, case management and providing support in individual’s homes.
Details of the position include:
Location: Danvers, MA
Program: PACT (Program for Assertive Community Treatment)
Schedule: Monday-Friday 8am to 4pm with one weekend per month, rotating holiday shifts
Start Date: Two weeks from offer
Salary: LPN - $2,300/week travel & $2,100/week local
RN - $2,750 week/travel $2,400/week local
*MUST have a Massachusetts license
Duties and Responsibilities (these are a general summary and not all inclusive):
Act as a primary case manager to 3-6 consumers
Provide nursing interventions to individuals
Assist in the management of medications and the overall functioning of the nursing department
Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload
Actively take part in supervision, staff meetings, in-services and trainings
Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.
Required experience in one of the following:
1 year experience with mental health/psychiatric patients
1 year of med administration
1 year knowledge of psychiatric medications, psychiatric and medical treatment
*Bilingual a plus!
