Job Summary: We are seeking a dedicated Registered Nurse (RN) to provide compassionate and skilled care to clients in their homes or facilities. As an integral member of our team, you will be responsible for assessing clients, implementing nursing care plans, and ensuring the delivery of high-quality services.

Essential Duties and Responsibilities:

  • Implement medical treatment plans and adhere to physicians' orders.

  • Maintain constant monitoring of clients, adhering to safety protocols at all times.

  • Assist in transporting clients to various activities, appointments, and community events as outlined in their Individual Service Plan (ISP).

  • Accurately document all activities and services in compliance with regulatory requirements.

  • Monitor clients' general health conditions and promptly report any medical issues to supervisors.

  • Provide skilled care such as G-Tube management and medication administration as per individual care plans.

  • Additional Responsibilities:

  • Adaptability to perform duties not listed above as assigned by management.

  • Required Skills/Abilities:

  • Professionalism, ethics, and respectfulness in all interactions.

  • Proficiency in providing personal care for non-ambulatory clients, utilizing equipment like Hoyer lifts, and facilitating transportation.

  • Excellent communication skills, both verbal and written, with strong interpersonal abilities.

  • Strong problem-solving skills and sound judgment in resolving issues.

Education and Experience:

  • Current RN licensure.

  • Valid CPR/First Aid certification.

  • Up-to-date TB screening.

  • Minimum of three (3) years of experience in providing skilled care to medically fragile individuals.

  • Flexibility to work varied hours as needed.
