State of Massachusetts ACCS Community Clinician in Brockton, Massachusetts

The Department of Mental Health's Brockton Multi Service Center (BMSC) is seeking an experienced Community Clinician for the Adult Community Clinical Services (ACCS) program. The successful candidate will be a part of a multi-disciplinary team that provides comprehensive services to individuals living in the community experiencing mental health and/or substance use disorder(s). The ACCS Clinician/Licensed Practitioner of the Healing Arts (LPHA) is responsible for leading trauma informed, person centered assessment& treatment planning activities and ensuring the interventions utilize evidence raised interventions &/or best practices, when clinically appropriate. Will provide face-to-face community-based services to assigned persons in their homes, homeless shelters, in hospital settings or any other setting in which the person served resides or currently requires services. Areas of focus will include engagement, assessment & treatment planning, risk mitigation, skill building, support through life & care transitions, health monitoring, integration with care coordination entities. _Please Note_: This position is 40 hours per week. The selected candidate will work on the first shift from 8:30 a.m. to 5:00 p.m. The days off are Sunday and Monday. * * _Duties and Responsibilities (these duties and responsibilities are not all inclusive):_ * Works as primary clinician for assigned persons served and is accountable for all service components provided by the team. * Responsible for the coordination and completion of the Comprehensive Assessment by meeting with the client, family and any other individuals that can provide information as well as by gathering and incorporating outside documentation. * Responsible the development, of person served Community Service Plan (CSP) in coordination with other service providers through meeting with the person served, knowledge of the person served history, in order to assist the person served in learning the necessary skills to live successfully in the community. * Conducts ongoing assessments; actively participates in discharge planning; assumes primary responsibility for developing, writing, implementing, evaluating, & revising overall treatment/recovery goals. * Serves on and provides clinical leadership to an integrated team which includes Group Living Environment(s) by attending regular team meetings, by coming prepared to present psychosocial history, mental status, diagnosis, treatment provided, and treatment planned for assigned cases. _Preferred Qualifications_: * Previous experience working with people with chronic mental illness and/or substance use disorder(s). * Bi-lingual preferred. _Special Requirements:_ * As a Licensed Practitioner of the Healing Arts, one of the following Massachusetts License(s) is required for this function: Physician, Registered Nurse, Psychologist, Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Social Worker (LCSW), Licensed Social Worker (LSW), Registered Occupational Therapist, Licensed Rehabilitation Counselor or Licensed Mental Health Counselor (LMHC). * Current and valid Massachusetts Class D Motor Vehicle Operator's license or equivalent license from the state in which you reside for travel. _DMH Mission Statement:_ The Brockton Multi Service Center (BMSC) provides comprehensive services to adults with mental health diagnoses.
