Commonwealth Care Alliance Licensed Practical Nurse (LPN) - Primary Care in Boston, Massachusetts

Why This Role is Important to Us:

Provides support in a clinic setting by managing patient flow through the clinic, obtaining pertinent clinical information including the patient's complaint, health history, and measurement of vital signs, preparing the patient for examination, and implementing patient treatment plans and therapies. Supports the care team in the overall coordination of the member's care.

What You'll Be Doing:

  • Rooming patients, monitoring patient flow through the clinic, and cleaning and preparing exam rooms between patients

  • Assisting patients before and after examination, including dressing and undressing, and positioning and transfers, which may include the use of a lift

  • Take and document patient history, and document standardized screening assessments such as a PHQ9.

  • Performing phlebotomy, giving immunizations, obtaining specimen samples, and processing labs for pick up

  • Assesses the health needs of patients under the supervision of the primary registered nurse.

  • Establishes plan of care jointly with medical doctor, primary care nurse, patient, and family.

  • Observes patients, charts and reports changes in patients' conditions, such as adverse reactions to

  • medication or treatment and takes any necessary action.

  • Administers prescribed medications, and notes times and amounts on patients' charts.

  • Measures and records patients' vital signs as required.

  • Provides basic patient care and treatments, such as wound care and other treatments as required.

  • Works as part of a health care team to assess patient needs, plan and modify care and implement

  • Participates in medical supply inventory management and ordering

  • Restocks supplies in exam room and monitors expiring supplies and medications for prioritized use and disposal

  • Interacts with vendors for receipt of deliveries, lab pick up, and laundry and biohazard pick up.

  • Participates in administrative patient care functions including follow-up phone calls and scheduling of appointments. Assists in scheduling patients for routine and/or urgent appointments. Assists in the completion of preauthorization forms, prescription renewals or other forms requested by clinicians. Faxes information to appropriated sources.

  • Collaborates with Registered Nurse on all cases to ensure delivery of quality patient care.

  • Assures accurate and timely documentation of care per company standards.

  • Works cooperatively with care teams and practice/medical assistant staff to ensure that all patient care needs are met.

  • Attends and participates in staff meetings and contributes to problem identification and resolution.

  • Keeps abreast of all changes by attending meetings and completing required online and in person trainings.

  • Provide clinical care to members via telehealth technologies (video, chat, etc.) for a clinically appropriate clinical care and care management services.

What We're Looking For:

Minimum Education Required

High School Diploma, GED or equivalent experience

Preferred Educational Experience

Associate degree

Minimum Years Experience Required

2+ years

Minimum Experience Knowledge, Skills and Abilities

  • Minimum of 2 years clinical experience

  • Must hold current LPN License to practice in Massachusetts

  • Current CPR Certification

  • Willing to learn and utilize telehealth technologies (video, chat, etc.), when appropriate, for a variety of clinical care and care management services.
