Griswold Home Care, Inc Care Coordinator in Berlin, New Jersey

Job Summary: Griswold Home Care of Camden County is seeking a highly motivated and compassionate individual for the role of Care Coordinator. This is a full-time administrative position in the healthcare industry located in Berlin, New Jersey. The ideal candidate will be responsible for coordinating care services for our clients and will serve as a liaison between clients, caregivers, and other healthcare professionals. This is an individual contributor role in which the Care Coordinator will work closely with our team to ensure high-quality care for our clients. Compensation & Benefits: The compensation for this position is $42,000 per year, paid biweekly. In addition, the Care Coordinator will be eligible for benefits including health insurance, paid time off, and retirement contributions. Responsibilities: • Coordinate and manage care services for clients, including scheduling and staffing of caregivers. • Serve as a liaison between clients, caregivers, and other healthcare professionals to ensure proper communication and coordination of care. • Conduct initial client assessments and develop individualized care plans. • Monitor and track client progress, making necessary adjustments to care plans as needed. • Maintain accurate and up-to-date client records and documentation. • Keep clients and their families informed and involved in the care process. • Provide ongoing support and guidance to caregivers. • Conduct regular quality assurance checks to ensure high-quality care and client satisfaction. • Comply with all state and federal regulations, as well as company policies and procedures.
